Abstract

The guidelines for the control of epidemics due to Shigella Dysenteriae 1 are divided into 5 sections. The 1st section introduces the organism. Shigella Dysenteriae 1 is an unusually virulent organism capable of causing widespread epidemics distinguished by high case fatality and extreme debility in survivors. The 2nd section reviews epidemics which have occurred in Central America, parts of Asia and Africa since 1968. Section 3 discusses relevant clinical and epidemiological information, such as, diseases caused by Shigella Dysenteriae 1, complications, and case fatality rates. The bulk of the guidelines are concentrated on strategies for prevention and control in section 4. Specific strategies for the control of Shigella Dysenteriae 1 epidemics consist of their early detection, proper treatment of cases, and surveillance and education of contacts. There is as yet no vaccine, despite many years of research. More general strategies during epidemics include ample provision of clean water, promotion of hand-washing with soap, improvement of personal and domestic hygiene assuring food safety and promoting breastfeeding, provision of facilities for the disposal of human excreta, prevention of fly breeding, and disinfection and disposal of dead bodies.
